You know how hanging folders come with those clear plastic things you use for labels? This is the same thing, but you print it on whatever color paper you want, laminate the whole sheet, trim it down with your paper trimmer and scissors, and then label with a Sharpie or a labelmaker. I use these in my own file cabinet and with clients.
Thicker laminating sheets are better if you plan to move the labels around a lot. I use the 3 mil thick laminating sheets, but the pointy ends tend to fray after relocating the label several times — fortunately mine don’t need to move so 3 mil works fine for me.
This is a simple username and password log with space to write in notes. Print it on some fun paper.

I created this printable music practice log for a guitar student a few years ago. The idea is to note your comfort level with a song or musical passage at the beginning of each week and to see how it improves with regular practice.
